Proctorship EVLA and TEVAR by dr. Taofan Sp.JP(K) at PJT Sanglah Hospital Denpasar

Taking place at the Integrated Cardiac Service at Sanglah Hospital, Denpasar, 13-14 May 2022, a proctorship activity in the vascular field was carried out by Dr Taofan Sp.JP(K) from Harapan Kita Hospital, Jakarta. This activity was carried out in order to transfer knowledge and increase the competence of staff in the vascular field at PJT Sanglah Hospital. The activity begins with an evaluation of cases for which action will be taken. There are 2 patients who will be treated according to their medical conditions. The first patient had a case of complicated aortic dissection with the TEVAR plan and the second patient with CVI on the Endovenous Laser Ablation (EVLA) plan. The action was carried out for a total of 10 hours and in progress the process went smoothly. After the procedure, the patient received treatment in the Intensive Cardiac Care Unit at Sanglah Hospital and the PJT Intermediate Ward at Sanglah Hospital for observation and evaluation.

EVLA is a method of treating varicose veins by utilizing laser light to restore the condition of swollen veins. If in the past the treatment of varicose veins was identical to a surgical procedure to remove varicose veins and leave a large incision, now varicose veins can be treated with minimally injured surgical procedures. This treatment is known as Endovenous Laser Ablation (EVLA) therapy. The wound that results from this method can be quite small, namely 1-2 mm which will disappear in one to two weeks. Varicose veins are not a trivial problem, varicose veins can even cause serious complications such as blockage of the veins that can cause pulmonary embolism, which can lead to death. Not only that, varicose veins can also cause wounds that are very difficult to heal.

TEVAR (Thoracic Endovascular Aortic Repair) Thoracic endovascular aortic repair (TEVAR) is a medical procedure to treat aortic problems. The aorta is the largest artery in the body. There are two kinds of aorta, namely those at the top (chest cavity) and the lower part of the body (abdomen/stomach). The aorta functions to drain blood containing oxygen and nutrients from the heart. When there is a disturbance in the aorta, all organs of the human body are disturbed. When the aortic wall tears, the consequences can be fatal. One of the most common aneurysms is in the upper (thoracic) aorta. An aneurysm occurs when the wall of the aorta weakens and widens or bulges like a balloon. TEVAR is classified as minimally invasive surgery. This means that the doctor only needs to make a small incision in the surgical procedure. With TEVAR, doctors use a device called a stent graft to re-strengthen the aorta. Stent graft is a device made of metal in the form of a tube covered with a special cloth. This device is inserted with a catheter. The point is to support the aneurysm of the aorta. This action is carried out with a comprehensive team approach consisting of intervention sub-specialists, other relevant specialists and medical personnel who are competent in their fields. TEVAR is the main choice for treating problems in the aorta with a lower risk consideration, compared to surgical procedures, where there is a high risk. Patients who undergo TEVAR can recover quickly. Patients can even return to their activities immediately after returning from the hospital.

By carrying out this proctoship activity, it is hoped that PJT Sanglah Hospital will continue to provide the best and up-to-date services for all patients who experience similar problems.
